Investigating high-risk rural regions for potentially preventable hospitalisations: a method for place-based primary healthcare planning

Summary. Rural communities face higher rates of preventable hospitalizations due to limited primary healthcare access. This paper develops a six-step method to identify high-risk regions and improve local healthcare pathways. The method examines service gaps, provider experiences, and patient journeys to recommend targeted interventions. Applied to ear, nose, and throat conditions in Australia, it provides a replicable framework for health agencies to plan equitable primary care services.
